ไข้ เจ็บคอ กลืนเจ็บ เป็นๆ หายมา 2 ปี tonsilar enlargement grade 3 no exudative ทำ throat swab cs ขึ้นเชื้อ K.pneumo,aeromonas hydrophilus sen cipro กินcipro 10 วัน ทำcs ซ้ำเจอ numerrous K.pneumo sen ได้AMK 10 วัน ทำcs ซ้ำ ขึ้น A.bum,Enterococus spp Resis amk,amoxy sen cef3 ขณะนี้on cef3 day 2 ขอบคุณครับ

ไม่แน่ใจว่าต้องการถามถึงแนว management รึป่าวนะคะ ขอแนะนำว่าให้ดู clinical เป็นหลักค่ะ เพราะส่วนใหญ่ common cause ที่ทำให้เกิด tonsillitis ส่วนใหญ่เกิดจาก gram+VE เช่นกลุ่ม Staph , Strep และ Gram -ve เช่น H. influ , Klebsiella ได้แต่น้อยกว่า หรือแม้กระทั่งเกิดจาก virus ก็ได้เหมือนกัน เช่น HSV , EBV เป็นต้น ดังนั้น ควรย้อนกลับไปว่ามั่นใจว่าเิกดจาก bacteria จริงๆหรือไม่ เพราะส่วนใหญ่ถ้าเปนจาก bacterial มักจะมาด้วย exudative
ส่วน 1st choice of antibiotics หากเป็นจาก bacterial ควรเป็น broad spectrum ที่ cover ได้หมด เช่น amoxy-clavulonic เป็นต้น การทำ throat swab นั้นไม่ได้เป็น routine ค่ะ ดู clinical เป็นหลัก
หาก treat infection ดีแล้ว แนะนำให้ refer ไปพบ ENT ดีกว่านะคะ ให้เค้าพิจารณาว่ามี indication ที่จะทำ tonsillectomy หรือไม่ เพราะนอกจาก recurrent infection บ่อยๆแล้ว indication อื่นๆ เช่น มีเรื่อง sleep disorder , dysphagia , peritonsillar abscess , มีกลิ่นปาก , unilat tonsillar hypertrophy ค่ะ
